Diagnostic Fee: $300

At your initial consultation with Dr. Zoneraich, Dr. Larsen or Dr. Peterson, the physician will review your medical history and records and determine what diagnostic testing is appropriate for your individual situation. While not all patients require every diagnostic test, a thorough diagnostic evaluation is critical in determining the ideal treatment plan, and thus achieving the ultimate goal of pregnancy.

At this same initial consultation, our front office team will be happy to assist you with estimated out of pocket expenses for your diagnostic testing.  For patients with insurance coverage, please keep in mind that we are providing a good faith estimate based on the information and determination of your benefits and coverage provided to us by your insurance plan however coverage and payment is governed by each member’s individual insurance plan at the time of actual claims processing.

Next Steps

After your claims have processed, if we have overestimated your out of pocket fees we will credit any cost difference towards your upcoming services, and if your plan reimburses less than was estimated, the difference will be considered patient responsibility and owed at your next visit or the next invoicing cycle whichever takes place first.

* For patients with insurance coverage for Fertility, please note that Arizona Advanced Reproductive Laboratories, LLC (AARL) only contracts with certain insurance companies. Payment for services rendered by AARL (including andrology, reproductive hormone testing, and embryology) for those not covered or partially covered through their insurance plan will be due at or prior to the start of treatment. An itemized receipt will be provided for you to submit to your insurance company for any potential reimbursement.

Single IVF Egg Freezing Cycle; $7,200

Not Included:

• Diagnostic Testing prior to Egg Freezing Cycle $500-$900 if self-pay
• Stimulation cycle medications $4,000-$5,250 (approximate)
• Yearly storage (after first year) $695
• Future oocyte thaw, ICSI fertilization culture of embryos, and Embryo Transfer $5,000-7,000
